Our Process
Care starts with a conversation. Here is exactly what happens between your first call and your first visit, no obligation, no pressure.
1. The first conversation
You call, or we call you back after you submit a request. A care coordinator listens to what is going on: what has changed at home, what worries you most, and what your loved one wants for themselves.
There is no charge for this call and no commitment. Many families use it simply to understand what options exist.
2. The in-home assessment
A registered nurse or care manager visits the home to look at the practical realities, mobility, medication, nutrition, fall risks, the layout of the house, and the routines that matter to your loved one.
- A room-by-room safety review
- A review of medications and existing care plans
- Discussion of daily routines, preferences, and personality
- An honest conversation about how many hours of support are actually needed
3. Your custom care plan
We write a plan that spells out what happens on each visit, who provides it, and how it will be measured. You approve it before anything begins, and it changes whenever your needs change.
4. Matching a caregiver
We match on skills first and personality second, both matter. Every caregiver of Cornerstone Caregivers is background checked, insured, and trained before their first shift.
If a match is not right, tell us. We will change it without a fuss.
5. Ongoing supervision
A supervisor checks in regularly, adjusts the plan, and keeps family members informed. You always have a phone number that reaches a person, not a queue.
