Skilled care at home

Where available, Cornerstone Caregivers provides licensed nursing in the home. Availability and scope are set by Georgia's nurse practice act. We will tell you exactly what we can and cannot provide during your assessment.

  • Wound care and dressing changes

  • Medication setup, administration, and reconciliation
  • Vital sign monitoring and clinical assessment
  • Diabetes management and blood glucose monitoring
  • Post-surgical care and drain management

Nurse oversight of non-medical care

Even when the day-to-day work is non-medical, a registered nurse oversees the care plan. The nurse performs the initial assessment, writes the plan, trains the caregiver on client-specific needs, and reassesses as the situation changes.

Hospital and rehab discharge

The first 30 days after a discharge carry the highest readmission risk. Nursing oversight during that window, medication reconciliation, symptom monitoring, follow-up appointment logistics, is one of the most effective ways to keep someone out of the hospital.
